Online

You can obtain a copy of a death certificate by filling out a form with the State of Arizona – Department of Health Services online and paying a $31.00 fee at the following website: https://azdhs.gov/licensing/vital-records/index.php#online-ordering-options .

In Person

You can request a copy of the death certificate in person.  (For more information see: http://www.azdhs.gov/licensing/vital-records/index.php – fees-home .) Note that at the time this article is written the in-person service is presently closed.

By Mail

You can request a copy of a death certificate by mail.  You will have to send payment, proof of relation to the decedent, and other information to the Department of Health Services.  Additional information is available at: https://www.cdc.gov/nchs/w2w/arizona.htm .

What You Need to Know if a Family Member or Loved One Was Killed in an Accident
